Note: The application process is fully online and there is a $20 application fee.
- To begin, fill out the application form then make your payment.
- Complete the application information and upload photos. You cannot save an incomplete application.
- Upon submitting the completed application, you will be redirected to a payment page.
- You will receive a confirmation email acknowledging the application receipt once submitted.
- This application is not a license agreement and does not constitute an offer to participate.
- Application is valid Jan 1-July 15 of the following year.
- This has to be done in one sitting. Be sure to have all of your information and photos ready.
